NWT Rental Office Forms for Landlords: Where to Find Them

All Forms Come from the GNWT Justice Portal

Every official form for residential tenancy matters in the Northwest Territories is published by the NWT Rental Office, a division of the Department of Justice. They are available at no charge from the GNWT Justice website.

Do not rely on an Ontario N12 or another province-specific notice under NWT law. For an eviction application, section 63(3) requires the approved form, and the GNWT publishes the NWT landlord application.

The Key Forms and Documents

Application to a Rental Officer — Landlord (form NWT9489/0326, March 2026 edition): This is the primary form for a landlord's tenancy application — non-payment, damage, personal use, breach, or termination. For a paper filing, GNWT instructions call for three copies of the form and supporting documents; the current form also allows secure-file submission. Available in English and French.

Application to a Rental Officer — Tenant (form NWT9490/0326, March 2026 edition): Filed by tenants. Landlords should be familiar with it in case a tenant files against them.

Standard Tenancy Agreement (May 2025 edition): The GNWT's optional template for a written tenancy agreement. Regulation section 1 says a written agreement may use the form in the Schedule. Available in English and French.

Inspection Report Form (September 2014): Used for entry inspections under section 15 and exit inspections under section 17.1. These reports document the condition of the premises at the beginning and end of the tenancy; section 42 allows a landlord to apply for repair costs for damage caused by a tenant's wilful or negligent conduct.

Abandoned Property Forms: The GNWT lists an Abandoned Personal Property form and Inventory form dated August 31, 2010, a Report of Sale dated December 11, 2014, and a Report of Return or Request to Dispose dated May 30, 2018.

Affidavit of Service Policy (April 2025): Not a form to fill in. It explains how the Rental Office prepares an affidavit for service of an order or reasons for decision, on request.

Security Deposit Interest Rate Schedule (January 2026): The annual schedule setting the rate at which landlords must accrue interest on security deposits held in trust.
